After filling out the booking form, you will be sent an email to confirm your address. Click to confirm, and you will be sent a copy of your signed document. Your form is not complete until you confirm your email. Don’t forget to also email a copy of your passport to judy@opulentquiltjourneys.com Please note that Adobe will not allow multiple signatures on the same form, so you will need to fill out a separate form for each participant if you are traveling with a partner.
